Carefully read the information below then follow the link at the bottom of the page to make your booking. Your virtual appointment is for 10 minutes. The final date and time to make your booking is 18th November 2020 at midnight, unfortunately no further bookings can be made after this time. Your booking is confirmed by email with email and SMS reminders 24 hours prior to your appointment.
Make sure you include your correct address information within your booking details. Your PSA test kit will be posted to you and without your full and correct address information this may not be possible.
On 19th November 2020 you will receive a Welcome Email plus a Teams meeting to discuss your results with your healthcare practitioner.
Expect your home PSA test to arrive no later than 23rd November 2020. Set aside ten minutes to administer this test and record your results on your instruction sheet. Have your results to hand to discuss at your Teams meeting with your healthcare practitioner.
